To change a child's name (who is under the age of 16) you simply need to complete and submit the application form below, which has provision for 4 children.  You do not need a debit or credit card to submit this form - you can pay by cheque or postal order.

If you do not have sole parental responsibility for your child, after you have submitted your application form, you need to send to us a letter of consent (by email, fax or post) signed by everyone with parental responsibility for the child (a link to an example of the letter we need will be emailed to you after your application has been reviewed).

Posting times are from when your letter of consent is received (if required).  If you have sole parental responsibility, the delivery times are from when your application is received.

        Signed-For airmail service.

        Posted same day using FedEx's International Priority service. Delivery is
        between 1 and 3 working days depending upon country (next working day to

Do you require our Legal Copy and Archive Service?

Organisations such as the Passport Office, DVLA, the financial institutions etc will not accept a photocopy of your Deed Poll document.  Our legal copies (also known as certified copies) will be accepted by these organisations in lieu of your original Deed Poll document thus avoiding your original Deed Poll being lost or damaged in the post.  Also, we archive an image of your signed Deed Poll document so you can order further legal copies in the future should you ever lose or mislay your Deed Poll.

Do you wish to claim a fee reduction because you are receiving a qualifying benefit?

You can claim a 10 fee reduction on the total value of your order if you are receiving either:
  • Income Support.
  • Income related Employment & Support Allowance.
  • Pension Credit.
  • Income-based Jobseeker's Allowance.
  • Housing Benefit.
  • Working Tax Credit.
  • Child Tax Credit (if your household income is less than 20,000 per annum).
  • Universal Credit (if your household income is less than 20,000 per annum).

The fee reduction is only available if you send us written evidence of your benefit entitlement (an original award notice, not a fax or photocopy, dated within the past 3 months).  If you need to obtain a new award notice click on this link (opens pop-up window) for contact details.  Please post your award notice to our address shown in section 10 below.
